Patient Medical Records and mammography films are available for pickup in the Health Information Management Department Monday-Friday from 9:00 am to 3:30 pm today through March 24.

Picture ID is required.

Records that as of March 24, 2006 are over eleven years old and pertain to patients who are at least 30 years old on that date, that have not picked up by March 24, 2006, will be destroyed.
